  • The application for the passport with all the documents must be submitted personally. It is not possible to apply via a third person or by sending documents by post.


    A person applying for a passport submits the following documents:



    1. Passport application filled out in Polish.
    2. Two (2) current passport photos sized 35mm x 45mm taken of the face with a natural expression, the eyes must be open and the mouth closed with no smile or other emotion present (if both ears are visible, this usually indicates that the subject is front-on to the camera, as required), without a head cover and without dark glasses; the face should constitute 70-80 % of the photo’s surface, the eyes 20-30 mm above the lower margin of the photo, the photo in color in high resolution, white background, with the technical requirements of minimum 300 ppi and 720 dpi.
    3. If a person who applies for a Polish passport does not have a PESEL numer, she or he must present original Polish birth certificate and consul will apply for PESEL number (see FORMS TO PRINT. ATTACHMENT 5). If this person is married/divorced/widowed, he or she also must present original Polish marriage certificate. It is not possible to issue a Polish passport without a PESEL number.
    4. Documents proving the eligibility for the reduction of the consular fee.
    5. Other documents, e.g.: document with the photo certifying the identity of the applicant.

    Additional information:


    • Please be aware that you may be requested to submit other documents such as: birth and marriage certificates, or documents proving Polish citizenship.
    • While applying for a passport you will be asked to present your current Polish passport. In case of loss of or damage to the passport, you need to submit official statement along with the application for the passport.
    • It is not possible to print the passport form from the Polish Embassy’s Internet site. Forms are available only in our office.
    • Polish passport is issued exclusively on the basis of Polish Registry Office’s documents. This means that people who have birth or marriage certificate issued by a foreign Registry Office, for example Israeli one, should transcript these documents.     • Polish passport contains biometric data: photo and fingerprints.
